Filling in application it is necessary to follow this rules and requests:

 

1. “ Customer” – necessary to name enterprise and it’s registration number;

2. “Address” – necessary to write Customers full juridical address, including post index;

3. “Contact person” – necessary to name responsible for application person’s phone and fax numbers;

4. “Date of application” – necessary to write application’s fill in date, month and year;

5. “Welder’s name, surname” – necessary to write welder’s name and surname;

6. “Welder’s personal code” – necessary to write welder’s personal code;

7. “Rules and regulations/Standards” – necessary to chose normative document for welder’s certification;

8. – 10. items must be filled in during the registration process of “Latvian Independent centre of specialists’ attestation and certification” application;

11. “Welding process” – necessary to chose one of named welding processes;

12. “Level of automatization” – necessary to chose on of named welding automatization levels;

13. “Base – metal” – necessary to chose on of named basic metal groups;

14. “Welding position” – necessary to chose one of named welding positions, as well as seam’s kind;

15. “Geometric parameters of billet” – necessary to write base metal density and snorkel’s outside diameter;

16. “Welding technology of collision seam” – necessary to chose one of named welding methods;

17. “Attestation of professional qualification” – necessary to name number of document and publishing date.
