Filling in application it is necessary to follow this rules and requests:

1. Name and surname of applicant;

2. Personal code of applicant;

3. Name of the company, the applicant is working in;

4. Full address of work place, phone and fax numbers;

5. Applicant’s work place’s e-mail address;

6. Applicant’s post;

7. Norms and rules/standards for applicant’s certification.

8. Areas and specializations of dangerous machinery: necessary must be marked with a tick;

9. Confirmations of educational and professional qualifications:

9.1. write in educational institution name, what gave qualification;

9.2. write in number and issuing year of special education certified document;

9.3. write in qualification attestations (documents, certificates, diplomas, etc.);

9.4. handing in application form, must be given such documents as:

9.4.1. copy of person identification document (passport or birth certificate);

9.4.2. copies of education, qualification and work experience attesting documents;

9.4.3. copy of definite example inquest of Prosperity minister about health situation for doable work;

9.4.4. copy of document about ending specialized professional training courses in sphere of dangerous machinery;

9.4.5. employers confirmation for taking part in dangerous machinery inspection, practical appraisal, results’ documenting and appraisal;

9.4.6. 2 photos;

10. Practise confirmations must be written in table (works connected with designing, making, repairing, modernization, exploitation, technical control and audits of dangerous machinery).

 

Applicant must hand in application and necessary documents for each area of dangerous devices separately.
